1. I acknowledge receipt and acceptance of Kore Defense Industries, LLP dba Kore Krav Maga (locally branded as Kore Self-Defense and Krav Maga) “Policies and Procedures”, accepted by me at the execution of this waiver with Kore Defense Industries, LLP. (please note page 2)

2. I understand that novel coronavirus (COVID-19) infections have been confirmed throughout the United States, including cases in Virginia and Loudoun County, VA. In accordance with the most recent guidance and protocols issued by the World Health Organization (WHO), the Centers for Disease Control and Prevention (CDC), the Virginia Department of Health, for slowing transmission of COVID-19, I agree that neither myself or family member shall visit or utilize the facilities, services, and programs of Kore Defense Industries, LLP (other than any exclusively online services and programs when/if available) if he or she (i) experiences symptoms of COVID-19, including, without limitation, fever, cough or shortness of breath, or (ii) has a suspected of diagnosed/confirmed case of COVID-19. (iii) has been exposed to a person with a suspected or diagnosed/confirmed case of COVID-19. The undersigned agrees to notify Kore Defense Industries immediately if he or she believes that any of the foregoing access/use restrictions may apply.

3. Kore Defense Industries has taken certain steps to implement recommended guidance and protocols issued by the Public Health Agencies for slowing the transmission of COVID-19. The undersigned acknowledges and agrees that Kore Defense Industries may revise its procedures at any time based on updated recommended guidance and protocols issued by the Public Health Agencies and further agrees to comply with Kore Defense Industries revised procedures. Kore Defense Industries, LLP reserves the right to take contact-free temperatures of everyone who enters the building, including children.

4. The undersigned fully understands and appreciates both the known and potential dangers of utilizing the facilities, services and programs of Kore Defense Industries and that use thereof by the undersigned and/or such participating children may, despite Kore Defense Industries and reasonable efforts to mitigate such dangers, result in exposure to COVID-19, which could result in quarantine requirements, serious illness, disability and/or death. The undersigned acknowledges the inherent limitations to social distancing associated with activities conducted in training with Kore Defense Industries, LLP.

5. The undersigned hereby agrees to indemnify, save and hold harmless Kore Defense Industries, its directors, officers, employees, volunteers and agents, and each of them, from any loss, liability, damages or costs they may incur (including injury/illness), whether caused by the negligence, active or passive, or otherwise while the undersigned or any participating child is in, upon or about the premises or any facilities or equipment therein or participating in any program affiliated with Kore Defense Industries.

1. No food or drinks (except water) are allowed on the mats.

2. All metal objects, jewelry, piercing, necklaces, or other such items should be removed before training.

3. No bare feet in the bathroom or area outside of the mat. Gym shoes must be put on inside the gym and taken off before leaving the gym. No outside sneakers are allowed on the gray mats unless the bottoms have been cleaned with disinfectant wipes. Outside sneakers can be worn on the blue mats and all non-matted areas.

4. Please avoid using perfumes, colognes, or heavy scents before and during training.

5. Do not train if you are sick! Stay home and get rest.. your training can wait.

6. If you have any type of bacterial or fungal skin infection, (ringworm, staph, etc.) you will not be allowed to train. They are highly contagious and they can quickly spread to other students. If you think you might have an infection, ask an instructor for clearance before training.

7. Please keep your training gear clean and organized. Make use of the cleaning wipes we've provided to prevent the spread of germs and bacteria from one piece of gear to another. If you can’t find them, or you see our supply is running low, let us know!

8. Kore is committed to providing a safe place for everyone to train free from all types of harassment, including, but not limited to; gender, sexual orientation, nationality, age, and disability.

9. There is no tolerance for any kind of sexual harassment or coercion, including, but not limited to; unwanted sexual advances, unwelcome physical contact, or targeted disrespectful/sexually explicit language.

10. If you feel that anyone at the gym (including instructors) has engaged in problematic behavior, please bring the issue to any instructor/owner with whom you feel comfortable discussing the issue (in person and/or email is best). We will investigate any reports and respond appropriately, while maintaining confidentiality. We are committed to ensuring our gym remains a safe environment for everyone.

11. Kore staff reserves the right to change these policies at their discretion.
